CHRONIC CARE CONDITIONS WE CAN HELP MANAGE

We have proven success working with patients with reversible disease states. Our Registered Dietitians produce positive results by reversing, or even eliminating, health issues such as:

DIABETES

Diabetes cannot be cured, but it can be controlled. Understanding the risk factors and symptoms is an important first step toward leading an active, healthy lifestyle while managing your diabetes.

Approximately 84 million American adults have diabetes. It is a serious health condition where blood sugar levels are higher than normal, but not high enough to be diagnosed as Type 2 diabetes. By making lifestyle changes in both nutrition and increased physical activity, you can manage your prediabetes and even prevent or delay Type 2 diabetes.

CHOLESTEROL

Nearly 1 in 3 American adults have high cholesterol. Too much cholesterol puts you at risk for heart disease and stroke. Cholesterol circulates in the blood and if your cholesterol levels rise, you will typically have other issues that negatively effect your health.

It is important to know your cholesterol numbers. You can improve your cholesterol by following a heart-healthy eating plan, being physically active, maintaining a healthy weight and, if needed, taking medication.

It is important to have your cholesterol checked and regularly speak with your doctor about cholesterol management.

HEART DISEASE

Coronary Artery Disease (CAD) is the most common type of heart disease in the United States. You can greatly reduce your risk for CAD through lifestyle changes and, in some cases, medication.

HYPERTENSION

Hypertension (or high blood pressure) is a common condition in which the force of blood against your artery walls is high enough that it may lead to health problems, such as heart disease, stroke, or kidney failure.

Known as the “silent killer,” nearly 1 in 3 adults in the United States is living with high blood pressure. Unfortunately, many adults can have high blood pressure without knowing it or showing any symptoms. It can be damaging to your heart, blood vessels, kidneys and other parts of your body, which is why it’s very important to get your blood pressure checked regularly.

TOBACCO USE

People who quit smoking can significantly reduce their risk of other smoking-related diseases and greatly reduce their medical expenses.

WEIGHT MANAGEMENT

According to the American Heart Association, nearly 70% of American adults are either overweight or obese, with obesity being defined as having too much body fat.1  People who have a body mass index (BMI) of 30 or higher are considered obese and are at a higher risk for health problems such as heart disease, stroke, high blood pressure, diabetes, and more.

GemCare Wellness can be utilized as an integral piece in the overall success of disease management. It is a population health strategy as well as an approach to personal health that requires knowledge sharing and building.
