DPP Terms and Conditions

I agree to the following:

• I have been diagnosed within the last 12 months with pre-diabetes, or, based on the risk assessment, I am at risk of becoming a person with diabetes, and, I wish to enroll in GemCare Wellness’s Center for Disease Control (CDC) recognized diabetes prevention program.

• I agree to attend a minimum of 16 diabetes prevention classes over the next year.

• I will make every effort to follow my coach’s recommendations.

• I have a desire to lose between 5 – 7% of my body weight.

• I agree to submit my weight prior to each online diabetes prevention classes, for tracking purposes of the program.

• I consent that GemCare Wellness will share my results and outcomes with the CDC and the Ohio Department of Health, for the sole purpose to determine if this program is helping to prevent diabetes.  My identity will remain confidential.

• I agree to share with GemCare Wellness my lab results, as they relate to this program and as they become available.
