*Program free of charge to participants. Funded by the Ohio Department of Health. If you have previously been diagnosed with type 1 or 2 diabetes, you are not eligible for enrollment in this program. This program is only for those diagnosed with prediabetes or at risk for developing type 2 diabetes.
